Merchant Statements (Attach 1 – 3 months of them. Full statements only since that is necessary to analyze the cost)

Since cost must be put in the proper context, please also explain the following: * Type of business you have
* How you accept cards (face to face, over the phone, online)
* The type of POS equipment you use (terminal, website, software, IP, dialup, etc)
* How many chargebacks have you had in the past 6 months?
* What’s the largest transaction size you have processed?
* Is the customer billed prior to the service being performed? If so, by how much time?
* Do you receive free rolls of paper or have any other perks with your current merchant service company?
